Please see below information about a survey to help the Lancashire County Council Health Improvement Team understand people’s experiences and interactions with the NHS Health Check in Lancashire. The results will help inform future delivery models to better enable the population of Lancashire to make positive lifestyle choices.

The Health Improvement Team are keen to hear about people’s experiences and interactions with the NHS Health Check in Lancashire, even if they have never taken up the opportunity of receiving the NHS Health Check.
